Case Presentation:

Upon arrival at the emergency department, the patient suffered a sudden cardiac arrest, requiring immediate action. Our skilled team sprang into action, initiating cardiopulmonary resuscitation (CPR) and administering lifesaving measures. After five cycles of CPR, we successfully achieved a return of circulation, giving us a glimmer of hope amidst the challenging circumstances.

Evaluation and Challenges:

An echocardiogram revealed a concerning left ventricular ejection fraction (LVEF) of only 10%, indicating significant cardiac impairment. Despite the administration of multiple inotropic medications and meticulous vasoactive pressure management, the patient’s blood pressure remained at 90/40, presenting a considerable challenge in stabilizing her condition.

Decisive Action:

With the potential for another cardiac arrest looming, our medical team made a crucial decision to pursue veno-arterial extracorporeal membrane oxygenation (VA ECMO) to provide mechanical circulatory support. The patient was promptly transferred to the Respiratory Intensive Care Unit (RITU) for the initiation of ECMO therapy, a complex procedure that required expertise and precision.

The Road to Recovery:

The initial days proved to be critical, as the patient’s heart functioned at minimal capacity. However, our team remained steadfast, providing continuous monitoring, meticulous care, and adjusting the inotropic support as needed. On the third day, a glimmer of hope emerged as the patient’s heart began to show signs of improvement, slowly regaining its strength.

With each passing day, the patient’s cardiac function continued to improve, albeit requiring higher doses of inotropic medications to sustain her progress. Finally, on the seventh day of ECMO support, our hard work and unwavering dedication paid off, as we were able to successfully wean her off ECMO.
